Is the anus protruding and red? The belly of the fish should be in a smooth line (look at one of your healthy fish).
Many of these symptoms can be together or separate.

4. Health
Does the discus look healthy? Discus with very dark coloration(depending on the strain) are probably sick or severely stressed. Check their anus. Are there white stringy poo? They are a sign of the presence of intestinal parasites.
Once they hatch they enter the umbrella stage, during which the larvae do not feed since they haven't yet developed a mouth or anus. They survive on their yolk sac during this time.

The technical term for what comes out the anus. This consists of the digested food remains and other products created by the fish which end up on the bottom of the aquarium. These by-products are the prime targets of proper filtration.
