To conduct a visual approach you must be on an IFR flight plan and have airport or traffic in sight with weather at least 1000 and 3. ATC can give clearance at an airport without reported weather if 1000 and 3 is expected.

Air Traffic Control will apply separation distances to IFR aircraft except to VFR following a medium or heavy aircraft or IFR on a visual approach and were the pilot reported the previous traffic in sight and follows and maintains own separation.

In a situation like the ATC would ask the VFR aircraft if they have their traffic in sight and providing the aircraft replied with having traffic ATC would say, 'maintain visual separation from that traffic'.
