Setup
Set up in a 2v1 to isolate the pick and roll.
The ball handler starts beyond the key with a ball in hand and a defender defending the ball handler.
The pick and roll is often employed by a shorter guard handling the ball and a taller forward or center setting the screen: if the taller defender switches to guard the ballhandler, then the offensive team can have favorable mismatches.

1 pass 2. The 1 - 2 and 4 are making a triangle. 5 sets a side screen.
3 and 5 are playing pick and roll. This option is only used after another option is played. The defense has to be broad in its motion.
Additional Pick and Roll Tips
When you're the one setting the pick, you can be called for a foul if you have your arms extended out from your body. You also can't move during the pick.
Another reason the pick and roll fails is because the screen is not wide enough. The screen, whether it be a front or back screen, should be set wide. The feet should be wider than the shoulders, yet the knees should be flexed to take the contact.

I use (4) lines as a warm-up drill and also use it as a way to teach players the pick and roll, shooting off the dribble while coming the pick - and just putting different priorities on the drill as I see fit.
