Osteonecrosis of the Jaw: Overview
Alternative Names: Ratner bone cavities, alveolar cavitational osteopathosis, Robert's bone cavity, trigger point bone cavity and interference field.

Bisphosphonate-related osteonecrosis of the jaw (BRONJ for short) is a condition in which bone is exposed in the upper or lower jaw, sometimes without cause. Symptoms may include pain, swelling and halitosis (bad breath).

The natural history of osteonecrosis is directly linked to the size and level of the necrosis. Very small lesions (involvement of less than 15% of the femoral head) may resolve without any further treatment.

Avascular necrosis is also called osteonecrosis, aseptic necrosis, or ischemic bone necrosis.
What causes avascular necrosis?

Osteonecrosis
Osteonecrosis means death of bone tissue. It is pronounced oss-tee-oh-neck-row-sis. It is also called aseptic necrosis, avascular necrosis or ischaemic necrosis. Osteo means bone and necrosis means cell or tissue death.

Osteonecrosis
Osteonecrosis is a disease that is caused when there is not an adequate supply of blood to a particular section of bone. When this happens the tissue in the bone dies off and this causes the bone itself to collapse.

osteonecrosis (avascular necrosis): bone death caused by lack of blood supply to the bone. A hip fracture can happen close to the network of blood vessels that go to the head of the femur.

Osteonecrosis of the Jaw
Osteonecrosis of the jaw (ONJ) has been reported in patients with cancer receiving treatment regimens including bisphosphonates. Many of these patients were also receiving chemotherapy and corticosteroids.

It is a heterogeneous disease characterized by the association of organomegaly (spleen, liver), bone disease (pain, bone infarcts, osteonecrosis) and cytopenia (thrombocytopenia, anemia and, more rarely, neutropenia).

Femoral neck fracture can disrupt the blood supply to the femoral head (avascular necrosis or osteonecrosis of the femoral head) with subsequent collapse and arthritis of the hip.

A rare side-effect from bisphosphonates is a condition called osteonecrosis of the jaw. This condition can result in severe damage to the jaw bone.

There have been a few reports of osteonecrosis (bone destruction) of the jaw in patients who take oral bisphosphonate drugs such as alendronate (Fosamax). These drugs are used to treat and prevent osteoporosis.

Biphosphonates and osteonecrosis of the jaw
A small number of cases of osteonecrosis of the jaw have been reported in people taking bisphosphonates for osteoporosis.
