(The risk is highest for patients with pancolitis, which involves the entire large intestine.) Cancer risk increases significantly after 8 - 10 years with pancolitis and after 12 - 15 years for left-sided colitis.

Oral aminosalicylates are used to treat mild to moderate extensive disease (pancolitis).
Mesalamine (Canasa, Rowasa) is placed in the rectum as a retention enema or suppository.

At its most severe, the entire colon can become inflamed (known as pancolitis). This form of ulcerative colitis is particularly challenging to treat.

The amount of increased risk depends upon the amount of inflamed colon and the duration of disease; pancolitis (inflammation of the entire colon) and colitis of 10 years' duration or longer are associated with the greatest risk for colorectal cancer.

When the inflammation occurs in the rectum and lower part of the colon it is called ulcerative proctitis. If the entire colon is affected it is called pancolitis. If only the left side of the colon is affected it is called limited or distal colitis.

Pancolitis (inflammation extending up the entire colon) causes additional problems of anaemia (often requiring blood transfusions), weight loss and poor nutrition. Medical treatment is essential in these patients.

In ulcerative colitis, the inflammation may extend into the upper parts of the colon. When the entire colon is involved, the terms pancolitis or universal colitis are used.

Other conditions affecting the appendix include carcinoids, cancer, villous adenomas, and diverticula. The appendix may also be affected by Crohn's disease or ulcerative colitis with pancolitis.
Etiology ...

Pancolitis affects the entire colon. This causes bloody diarrhea, pain and cramping in the abdomen, weight loss, night sweating and fatigue. Fulminant colitis is the rarest form of ulcerative colitis and the most life-threatening.
