Even if chemotherapy results in a complete clinical response (ie, normal physical examination, normal serum CA 125, and negative CT scan of the abdomen and pelvis), about 50% of patients with stage III or IV cancer have residual tumor.

Sometimes during brain surgery to treat acoustic neuromas, not all of the tumor can be safely removed, and some residual tumor must be left behind. Radiosurgery is often used post-operatively to treat residual tumor in these cases.

If a significant amount of residual tumor is left in the neck, external radiotherapy has been indicated and has proven useful especially in those cases when residual tumor does not take up radioiodine.

Radiosurgery to deliver additional radiation to residual tumor masses is also being used in specific cases.

First the tumor and the skin are removed and then studied under a microscope to see if any residual tumor is left. If so, the remaining cells can be excised immediately.

The purpose of radiosurgery is to stop the growth of a small tumor. It also may be used for residual tumors, portions of a tumor that traditional brain surgery can't remove without damaging brain tissue.

Researchers are exploring the use of chemotherapy before surgery to help shrink the tumor and after surgery to help kill residual tumor cells. Chemotherapy is given in cycles with intervals of several weeks depending on the drugs used.

One in 3 individuals may be left with residual tumor after surgery, with recurrence rates as high as 30% to 50%. The 5-year recurrence rate for malignant meningioma is 65%. Radiation therapy can be used in managing recurrent tumors.

Total Body Irradiation (TBI): Radiotherapy often given in several doses prior to bone marrow transplantation with the aim of killing residual tumor cells in the patient. It is used in conjunction with high dose anti-cancer drugs.

Salvage chemotherapy (fluorouracil with cisplatin plus a radiation boost) may avoid permanent colostomy in patients with residual tumor following initial nonoperative therapy.
