Tenesmus
Definition
Tenesmus is the feeling that you constantly need to pass stools, even though your bowels are already empty. It may involve straining, pain, and cramping.

Tenesmus usually occurs with inflammatory diseases of the bowels. These diseases may be caused by an infection or other conditions.
People with tenesmus feel as though they need to pass stool. Although they strain, they pass little stool.

Shigella dysentery (bacillary dysentery) occurs quite suddenly and typically causes many (10-25) bloody episodes of diarrhoea a day, a high temperature, gastric pain, and pain on defaecation (tenesmus).

A feeling of wanting to go to the toilet but with nothing to pass is also common (tenesmus). Water is not absorbed so well in the inflamed colon, which makes the diarrhoea watery.
Blood mixed with diarrhoea is common (bloody diarrhoea).

Signs and symptoms include colicky abdominal pain, bloody diarrhea, tenesmus, steatorrhea, weight loss, and nausea and vomiting. Less common are bowel obstruction, fistulas, bowel perforation, and massive rectal bleeding.

The discharges from the bowels succeed each other with great frequency, and the painful feeling of pressure downwards (tenesmus) becomes so intense that the patient is constantly desiring to defecate.

In patients with mild to moderate inflammation, symptoms can include rectal bleeding, diarrhea, mild abdominal cramping, stool urgency, and tenesmus (discomfort and the feeling that you have not completely emptied your rectum after a bowel movement).

Did individual have symptoms such as fecal incontinence, rectal spasms, tenesmus, or bleeding or mucus discharge from the anus?
On physical exam, did the physician find a large red mass with circular (concentric) folds protruding from the anus?

People may have a sensation called "downward urge," also known as "tenesmus," or the feeling that a bowel movement will relieve their discomfort.

Symptoms of tenesmus or incomplete evacuation suggest rectal inflammation.

Rectal pain during the diarrhea (tenesmus) is also common. The diarrhea may contain blood or mucus. Usually, the illness lasts about 2 weeks, but it can come back again and again if not treatment is given.

Ulcers form in places where the inflammation has killed colon lining cells. The ulcers bleed and produce pus and mucus. You may have abdominal pain, diarrhea, rectal bleeding, painful spasms (tenesmus), lack of appetite, fever, and fatigue.
