It should have one main leader or central trunk. On the tree here, the leader is a bit curved, from not being staked and bending in the wind. This is OK, but the side branches on the leader will have to go.

Do not pile mulch against the trunk. For established trees in lawns, create a circle of mulch about 2 feet in diameter for each inch of trunk. Try to apply the mulch at least 6 inches beyond the drip line of the tree.
There should only be one main trunk per tree. If there is a secondary trunk, where the two meet (the crotch) will weaken and the tree may split. The split is an excellent place for insect infestation.
