Informed Consent
The most basic of rule of medical care is that the patient has an absolute right to be informed about the proposed treatment before agreeing to it; this is known as "informed consent.
Informed consent
A contractual agreement between a patient and a physician, whereby the patient gives permission to undergo a certain procedure based on as clear an understanding of the issue as is possible.
Informed Consent - Voluntary permission given by a patient of patient's legal representative (parent, guardian, etc) for medical, surgical or experimental treatment.
informed consent form - a form signed prior to surgery by the parent(s) or legal guardian(s) of a child under age 18, or by a patient 18 years of age and older, which explains everything involved in the surgery, including the possible risks.

What is an informed consent form?
It really comes down to informed consent.
It's not about knowing it all, or tuning out information or a scenario you don't want to hear. It is about being provided all of the options and information, and making your own decisions and your own choice.
It can occur at any age and can follow a natural disaster such as flood or fire, a man-made disaster such as war or imprisonment, rape, or assault (such as a traumatic birth experience, often without acceptable informed consent).
