The kidneys become 'leaky' and in pre-eclampsia there is protein (also known as proteinuria). You will see that it is sometimes documented as +, 2+, or 3+.

Your midwife will be looking out for a combination of raised blood pressure, worsening proteinuria and sudden swelling of the hands, feet and face, all of which suggest pre-eclampsia. Sometimes only one of the symptoms is present.

If protein loss exceeds 300 mg in 24 hours, this is considered proteinuria. Urine dipstick analysis for protein measures only a single point in time and does not necessarily reflect protein loss over 24 hours.

Protein in the urine (proteinuria). 300 mg or more per 24-hour period or at least 1+ on dipstick in at least two random urine samples collected at least 6 hours apart.

Preeclampsia is defined as the combination of high blood pressure (hypertension), swelling (edema), and protein in the urine (albuminuria, proteinuria) developing after the 20th week of pregnancy.

Preeclampsia/high blood pressure: Preeclampsia means the development of hypertension with edema or proteinuria or both during pregnancy. It generally occurs after twenty weeks of gestation. It is also called toxemia.

The two main signs of pre-eclampsia are high blood pressure (hypertension) and protein in the urine (proteinuria). Swelling (oedema) is not always associated with pre-eclampsia as it may be part of normal pregnancy.

Key signs are increased blood pressure and protein in the urine (proteinuria). Other symptoms that seem to occur with preeclampsia include persistent headaches, blurred vision, sensitivity to light and abdominal pain.

At this point it can only be detected by checking blood pressure (which may be raised) and testing the urine for protein (known as proteinuria).

Lab results indicating elevated uric acid and/or protein in the urine (proteinuria).
Swelling of the hands and face that does not go away during the day.

These tests include serum creatinine and blood urea nitrogen (BUN); elevated levels of either can indicate kidney damage. Proteinuria, an excess of protein in the urine, is also a sign of kidney disease.

These toxins act on the vascular endothelium, causing hypertension and proteinuria. If severe, it progresses to fulminant pre-eclampsia, with headaches and visual disturbances.

Common risk factors associated with the condition are advanced maternal age, a woman experiencing her first pregnancy, proteinuria in the first trimester of pregnancy and deformities in the developing fetus.
